COLEGIO DE SAN AGUSTIN TERMS & CONDITIONS


1. All participants are required to fill out the following forms upon enrollment:
• Student Information Sheet
• Swimmer's Goal Sheet
• Liability Waiver & Photo / Video Release Form

2. Aqualogic Swim Co. is offering the following swim packages at Colegio San Agustin:

Two times a week
• 8 lessons - valid for 1 month only
• 16 lessons - valid for 2 months only
• 24 lessons - valid for 3 months only

Once a week (AquaBabes , AquaKids, and AquaFitness only)
• 8 lessons - valid for 2 months only

3. An annual registration fee of P500 per student will be charged by Aqualogic Swim Co upon reservation. Please deposit registration fee to BPI Current Account 8241-0041-72 and fax deposit slip to 837-1716 or email to this address.

4. Tuition fees should be settled in full before the first lesson to secure placement in desired class. Payments are accepted in CASH, CHECK payable to AQUALOGIC SWIM CO.

5. All students will be issued a receipt and will be required to fill out the all the information on the Class Card upon enrollment. Please submit the class card to the Aqualogic Coordinator / Instructor on the first lesson.

All students should secure their class cards from the Aqualogic Coordinator / Instructor before each swim lesson.

The instructor and the student or guardian will sign the card to monitor attendance.
Please return the Class Card/s to the Aqualogic Coordinator / Instructor after signing. NO CLASS CARD, NO LESSON.

6. Classes will start promptly at the designated time. We require students to arrive 10 minutes before the scheduled lesson. All students must be already in their swim attire and have applied sunscreen prior to their arrival at the pool.

7. Students who arrive 10 minutes late (AquBabes)/ 15 minutes late (AquaKids/AquaTech) will no longer be admitted to attend the lesson for that hour as it disrupts the flow of instruction.

8. Each student is only allowed to be accompanied by maximum of two (2) adults to avoid overcrowding in the pool area.

9. Students, parents or guardians may not be at the pool area more than 30 minutes prior to the class designated start time.

10. Students, guardians, and or yaya's are not permitted to loiter around the school area and must stay at the designated area at all times.

11. Students are required to bring their own towels for their personal use.

12. Littering is strictly prohibited. Students, parents, and Aqualogic Staff are responsible to dispose their trash in the designated garbage bins.

13. No outside food and beverage is permitted at the pool area.

14. Only the outside pool bathrooms may be used for students, parents, or guardians, etc.

15. Students, parents or guardians, including personal security, yaya's, and drivers may not remain at the pool area more than 15 minutes after the designated class ends.

16. Should the student decide to be absent due to personal reasons, Aqualogic should be advised at least one hour prior to the scheduled swim class. Failure to do so will be considered 'No Show' and the lesson will be forfeited/deducted against the total number of lessons in the swim package.

17. Students are expected to attend the scheduled class they signed up for, and should inform Aqualogic if there are any changes in schedule, otherwise we will not be able to accommodate the student without prior notice and consent from Head Instructor.

18. Should the lesson be cancelled due to bad weather or unavoidable circumstance, the student will be informed at least one hour prior to the schedule swim class. The cancelled class will NOT be deducted against the total number of lessons in the swim package.

19. Aqualogic has the right to refuse admittance of students who will disrupt the harmonious class environment.

20. Aqualogic Swim Co. has a NO REFUND POLICY for cancellation of classes after the start of the swim package. Unused swim lessons may be transferred to another student upon the approval of the Head Instructor and should be consumed within the validity period.
